Reminder: HEDIS MY 2024 Medical Record Requests
Home State Health staff began sending medical record requests to provider offices in late January 2025. If you received a medical record request and have submitted the requested documentation, we appreciate your assistance.
If you have not submitted the requested medical record information, please consider this as a reminder to reply to our request promptly. Please be sure to follow the submission instructions outlined in our request. Everyone's assistance is crucial to the success of this project and your prompt response will ensure that Home State Health measures accurately represent the high quality of care you provide to our members.
