Your Healthy Transition to Independent Living

As a teen preparing to transition out of foster care, we know you may face many obstacles on your way to adulthood.  We are here to help. The Adolescent to Adult (a2A) Program is designed especially for you.  

 

    Who is eligible for the Show Me Healthy Kids Adolescent to Adult Program?

    Show Me Healthy Kids (SMHK) Adolescent to Adult, or "a2A" program is for eligible SMHK members who are: 

    • At least 16 years old AND
    • Aging out of foster care OR
    • Those who have a change in eligibility for SMHK

    How Can You Contact a Member of the SMHK Care Management Team?

    The SMHK Health Plan has a Care Management Team that is able to support you in many ways as you transition in to adulthood.

    What Kind of Support Can You Get?

    • Understanding and accessing care for your health and wellness needs
    • Learning your healthcare rights and how to take an active part in your healthcare decisions
    • Helping make your own decisions about your care and medication
    • Managing your mental health, including anxiety, depression, loss, and loneliness
    • Understanding how to ask questions or share things you are worried about with your provider
    • Asking for help about substance use 
    • Accessing health insurance coverage once SMHK ends
    • Understanding health information shared by providers

    Learn about Advance Directives and how to identify a support person who can make health care decisions for you, if you are unable to do so.

    Receive assistance with problem solving and finding resources around
    independent living needs, such as:

    • Housing
    • Education
    • Employment
    • Mentoring
    • Insurance options
    • Health plan benefits
    • Career development

    Understanding and Accessing SMHK Benefits

    Access to the HiSET, a high school diploma equivalent that may help you get a better job, join the military, attend career trainings, or continue education.

    Provided to members at risk of failing one or more core subject areas or for those who are preparing for the HiSET/GED.

    We will work with you on improving your health through reducing substance use through education, planning and accessing a provider.

    A program designed to assist you in maintaining safety during a suicidal crisis through assessments, safety planning and follow up phone calls.

    Access to an app that includes both engagement activities and the option to connect with a staff member who can help manage loneliness.

    Telehealth services provides 24-hour access to in-network providers for non-emergency health issues.

    • Get medical advice, a diagnosis or a prescription by phone or video.
       

    We can arrange non-emergency medical transportation to select medical and health services. 

    Earn rewards with My Health Pays when you complete healthy activities like a yearly wellness exam, annual screenings, tests, and other ways to protect your health.

    Our Start Smart for Baby program was created to support you when you need it, from pregnancy to delivery and after your baby is born.
