Home State Health offers Care Management, a no cost benefit to you, to support you with meeting your health goals and supporting you on your health journey. Our care managers are registered nurses or behavioral health clinicians that can help you with the following:
- Health Condition Support - Get help understanding and managing your health conditions
- Provider Assistance - We can help you find doctors and specialists
- Care Team Collaboration - We work with your healthcare provider to support your care
- Education and Resources - We will discuss benefits and resources available to you to help meet your needs
To get signed up for services, contact Care Management at 1-855-694-HOME (4663) (TTY: 711) or 1-877-236-1020 for Show Me Healthy Kids, or fill out the Care Management Referral form below to self refer and a member of our team will call you.
Members engaged in Care Management Services may qualify for additional programs and benefits.
Listed below are a few of the programs you may qualify for. If you are interested in any of these programs please contact us to get started.
Asthma Waves Program
The asthma program gives you an asthma care manager who will talk with you by phone, teach you about managing asthma, and provide support. They help you follow asthma treatment plans, avoid flare-ups, and stay as healthy as possible. They can also connect you with other helpful services.
You will also receive a spacer, peak flow meter, and hypoallergic item when you enroll in care management with asthma.
Behavioral Health Care Management
Behavioral Health Care Management is here to support your mental well-being. We focus on your strengths and overall health, not just treating illness. Good mental health means feeling well and making healthy choices.
If you have conditions like depression, anxiety, bipolar disorder, schizophrenia, or substance use concerns, you can get help through our care management program. A care team member will check in with you, share helpful information, connect you to resources, and offer support.
Enrolled members may be eligible to receive:
- Medication Lock Boxes
- Sound Machines
- Fidget Spinners
General Pediatric & Adult Care Management
Our Pediatric & Adult Care Management team is here to help you stay as healthy as possible and live your best life. Your care manager will work with you, your family, and your doctors to set goals and support you in reaching them.
Diabetes & Hypertension (Virtual View Program)
If you have a history of, or are diagnosed with, high blood pressure (hypertension) or high blood sugars (diabetes) we have a program available to you. You can receive a blood pressure monitor or glucose monitor at no charge to you that is delivered directly to your door. You are eligible for this program if you are pregnant as well.
Lead Care Management
If you or your child has an elevated blood lead level, you are eligible for care management services. A dedicated care manager will provide you with education about lead exposure and how to reduce your child’s lead levels.
Pregnancy & Postpartum Support - Start Smart for Your Baby
Prenatal care is an important way to keep you and your baby healthy during your pregnancy. As a Home State Health and Show Me Healthy Kid member, you have access to our Care Management Program, Start Smart for Your Baby, that will provide you with all the resources you need, like:
- Personalized care based on your stage of pregnancy
- Telephonic or digital care management available
- High Risk Pregnancy care management and education
- Help with community resources, such as housing, food, clothing and more
- Rewards for completing pregnancy screenings
- Help with obtaining a breast pump
- Referral assistance for mental health and substance use
- Birth control planning after delivery
- Referral assistance to community programs

Sickle Cell Care Management
Taking control of your health is important in managing sickle cell disease. This program gives you the tools and support needed to focus on your health. It helps you build skills and confidence to manage your sickle cell disease.
Weight Management
If you are interested in losing weight and are ready to take a step towards a healthier you, we can help. Our care managers will support you with developing a weight loss goal and providing education and support to reach your goal. You may be eligible for Weight Watchers vouchers through this program.

Additional Member Benefits
- 24-Hour Nurse Advice Line for nurses to answer health-related questions.
- Connections Plus Phones: Members ages 18 years or older, enrolled in complex care management, and who don't qualify for a SafeLink Wireless phone may be eligible to receive a free phone through the health plan.
- Digital Care Management: An app called Wellframe that connects members with the Home State Health Care Management team for support.
- Health coaching program for members with asthma, diabetes, depression, or pediatric obesity, and more. Age restrictions may apply.
- HiSet Voucher: Covers the cost of taking the MO High School Equivalency Test (members ages 16+)
- Homer Helps ADD/ADHD: Members ages 12 and under with an ADD/ADHD diagnosis and prescribed medication for treatment may qualify to receive an emotional support kit.
- If pregnant: Home visiting program referrals, breast pumps, belly bands, and rewards for attending OB appointments. See Pregnancy Page for details.
- Medical Alert Tags: Members with a diagnosis of asthma, epilepsy, autism, developmental delay, or allergies may qualify to receive a medical alert tag.
- Mom’s Meals: Members can receive up to 30 days of home meals after giving birth or being released from the hospital.
- Rewards for completing healthy behaviors
- Telehealth-24/7 access to virtual visits with U.S. licensed doctors.
- Transportation for doctor appointments, lab tests, and any other non-emergency medical appointments (if eligible for benefit).
